The undersigned member, parent or guardian do hereby acknowledge the dangers inherent in waterskiing, water shows and water related sports. I understand that at this event or related activities. I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose. In consideration of the participation as a member of the Aquanut Water Shows, Inc. for the season of January 1, 2026 through December 31, 2026. I do hereby, for myself, my heirs, executors and administrators and/or for the minor for whom I am signing; waive, release and forever discharge the following organizations, their officers, employees, agents, subsidiaries and affiliates of:
Aquanut Water Shows. Inc. The Village of Twin Lakes, Wisconsin; the Park Board of the Village of Twin Lakes, Wisconsin; any of the Aquanut Water Shows, Inc. sponsors and contractor’s, USA Water Ski organization; Wisconsin Water Ski Federation, exhibitions and/or any of the Aquanut Water Shows, Inc. organized events of and from any and all rights, claims, and demands for actions of any and all loss, damages, injury or death sustained to me, or by the minor(s) for, whom I am signing through my participation.
